What is ESI Form 7b
ESIC form 7b also known as medical acceptance card. It is a request form of insured employee who wants to include him/her under particular doctor for treatment in ESIC hospital / dispensary. This is helpful when you want a treatment from a doctor whom you think best for your problem.
ESI Form 7b is a 2 part letter. One part consists of insured person details and other part consists of details of particular doctor from whom the insured person wants to get treatment.
How to Fill ESI Med 7b Form
You have to fill your personal details under medical acceptance card first part. In the second part, you need to write the doctor name in a dispensary.
In first part, write your name, father / husband’s name, factory name, present residential address for communication and then sign under. Below that write your insurance number.
In second part, write the doctor name, date and then sign under. Leave the later part which will filled by doctor.

How to Download ESI Medical Acceptance Card Online
ESI medical acceptance card can be downloaded from online ESI employer portal. Your employer need to login using employer login id and password. If you are working in a small organization where HR team will not be strong and may not know all this procedures. In that case you can help them to get your med 7b card.
Step 1: Login to ESIC Employer Portal.
Step 2: Under employee, you can find print counter foil link. click it.

Step 3: Now, you need to enter ESIC IP number of the insured person and click on the print counter foil.
Step 4: Now employee print counterfoil screen will be displayed. On this page you can find a link titled ‘Medical Acceptance Card (Click here to view medical acceptance card)’. Once you click that link, ESIC Medical Acceptance Card Form 7b will be downloaded.
Along with this form, insured person should also submit following documents as well.
- ESIC Card’s Xerox attested by employer.
- E-Pehchan Card or Temporary Identity Certificate with Aadhar card number attested by employer.
- Entitlement to Benefit Certificate
Usually to get treatment from ESI hospitals, ESIC pehchan card or temporary ID card is enough. Medical acceptance card is useful only when the insured person or his dependents suffer from long term illness, then you have to submit ESI Form 7b.
